Becoming A Patient At Regen IV Wellness

A new patient appointment with our nurse practitioner is REQUIRED to establish you with our practice and clear you for treatment and prescriptions. The free consult does not qualify as a new patient consultation and is for informational purposes only.

An annual consultation to discuss updated medical history and health status with our nurse practitioner is REQUIRED to maintain active patient status and receive treatment or prescription refills.

Cancellation Policy

24-hour cancellation is required for all services. No show appointments will be assessed a $100 charge with the credit card on file. Cancellation must either be done in person, or by phone, text or email

Emergency Policy

For emergent, after hours calls, holidays, or weekends, call the office number, leave a message and someone will get back to you at the earliest possible opportunity. If you are in serious emergency, please call 911 or go to the nearest emergency room.

Payment Policy

Payment is due at time of service. Telemedicine visits require a credit card on file to charge after the visit. No show appointments incur a $100 no show fee.

​We accept cash, check and credit card. Checks are made out to Regen IV Wellness. A $35 service charge will be added to all returned checks.

​Regen IV Wellness is not contracted with any insurance companies and is not allowed to submit any claims on patient’s behalf. Regen IV Wellness provides documentation for insurance filing upon patient request.

I understand that I am financially responsible for any charges incurred and must pay Regen IV Wellness up front and in full the same day that I receive treatment.

I understand that is my responsibility to contact my insurance carrier directly regarding any claims I would like to make and that I must submit these claims myself.

I understand that any payments or reimbursements from any insurance company will come directly to me. I hereby authorize Regen IV Wellness to release any medical information necessary to process claims I may have submitted for services/treatments in the office.

If full payment is not made for services rendered, I understand that Regen IV Wellness may be forced to take collection or legal action. I agree I will be liable for additional expenses, such as legal fees, interest on the unpaid portion of my account balance, and collection fees.

Returns, Exchanges, and Refund Policy

Expired and/or opened products are not eligible for a refund. Unopened product can be returned.

Items damaged in transit are eligible for a replacement item. Replacements will be shipped once the original item is returned.

Memberships and programs may incur a cancellation fee. Refer to Commitment Agreement when registering for these services.
